Highlights of the CSEA-Sponsored Classic Disability Income Plan:
- Payroll Deductions: Members may elect to have their premiums deducted through payroll-no invoice, no checks, no hassles!
- Benefit Period: Up to 24 months for a covered sickness or injury.
- Waiting Period: A choice of 0 days for accident / 7 days for illness, or 30 days for both accident or illness.
- Benefit Amount: Up to $3,000 per month based upon annual salary.
- No Offsets or Coordination of Benefits: This plan pays regardless of sick leave pay, workers compensation or any other insurance payments.
- In-patient Hospital Benefits: This plan can pay an additional daily benefit if an insured is confined in a hospital as a patient as a result of a covered disability.
- Pregnancy: Up to 1 month of benefits will be paid if disabled due to a covered pregnancy.
- Payment For Medical Expenses: Will pay up to the monthly benefit amount for the treatment of minor, non disabling injuries caused by a covered accident.
- Waiver of Premium: If an insured member becomes totally disabled for 6 consecutive months, all future premiums will be waived as long as the member continues to receive benefits.
- Vocational Rehabilitation Benefits: Approved candidates can receive up to 24 months of a personalized vocational program.
- Survivor Benefits: Up to 2 additional months of benefits for the insured's family, if the member dies while receiving benefits for a covered disability that lasted at least 30 days prior to his/her death.
- Work Stoppage Benefits: An insured member will pay no premiums and coverage will continue if a CSEA authorized work action results in a stoppage of his/her pay.
- Accidental Death Benefit: $5,000 of coverage is included in an insured's policy at no extra cost.
- Optional Accidental Death Benefit: Up to $100,000 of coverage is available for an extra cost.
- Optional Lifetime Benefit: For an additional premium, this benefit will pay up to lifetime for total disability due to a covered off the job accident.

Who is eligible to apply for this Disability Plan?
All CSEA members under age 60 who are regularly and actively performing the duties of their occupation according to their regular schedule on a full time basis.

How much monthly benefit is available?
Depending upon your salary, members can apply for up to $3,000 of monthly benefit.

How long do I have to wait before I begin to collect benefits?
Benefits begin after your waiting period. You can choose either the 0 day for accident/7 day for illness waiting period or the 30 day waiting period for both accident or illness.

What is the maximum benefit period for an insured to collect benefits?
The maximum benefit period for sickness or accident is 24 months, unless an insured selects the lifetime benefit period option for off the job accidents.

Is my spouse also eligible for this coverage?
Unless your spouse is working full time, under age 60 and a member of CSEA, he/she is not eligible for this coverage.

Is there any pre-existing condition limitation?
Yes. Any condition for which medical advice was given, or treatment was recommended by or received by a physician during the 6 month period before the effective date of coverage, will not be covered until coverage has been continuously in effect for 12 months.

As an insured member, will I continue paying premiums if I become totally disabled?
If an insured member has been totally disabled for 6 consecutive months, all future premiums will be waived for as long they continue to receive disability benefits.

For how long can I continue to be insured?
Your insurance will continue as long as you pay your premiums, you are a CSEA member, continue to work full time and you do not enter active duty in the armed forces and the Group Policy remains in effect.

Will my premiums ever go up?
Yes, rates increase following the attainment of a new age bracket i.e. 30, 40, 50, etc. The premium rate table is not guaranteed. Rates may be changed by New York Life on any premium due date and on any date benefits are changed.
